Pre-Hirudotherapy Visit Requirements
Before you schedule a consultation or visit at ALA-MED HIRUDOTHERAPY & ACADEMY of HIRUDOTHERAPY locaions, we have to have your answers to the listed information below. Please forward your answers to ALA-MED via Fax: 888-825-0793 or via e-mail attachment to ala@amazingleeches.com .
The very first step, however, is to read the “Contraindications &Conditions” on the company website - www.amazingleeches.com . Please let us know immediately if ANY CONDITIONS from the provided list apply to you. If none are adequate to your health status please continue on.
- Please describe all of your health problems and complaints;
- Include all available copies of medical diagnosis, diagnostic tests, MD readings, etc.;
- List all medication you are taking;
- List all therapies, diets, body cleansings you are involved with at the present moment;
- State your age, height and weight;
- Submit a copy of your most recent blood test (no older than 30 days) or obtain a new one.

Appointment Cancellation and Booking Policy:
In advance of your consultation we spend considerable time preparing for your visit,
reviewing your case notes and reserving the medicinal leeches.
We reasonably assume that when you book an appointment with our specialist
you genuinely plan to attend on the agreed date and time.
For this reason we ask that you place a deposit of 50% of the Initial Consultation Fee*
at the time of making your appointment. When you attend your first consultation or visit, your payment
will be reduced by the full amount of your deposit.
* Consultation Fee: $90,-
Appointment cancellations and rescheduls:
- giving less than 24hrs written notice by email or phone call during the business hours to our clinic
will incurr a fee of half of your deposit;
- if called or e-mailed notice of cancellation or rescheduled in the timely fashion, the deposit will be
applied to the next appointment or fully refunded;
- any missed appointment without a valid notice given will aloud the clinic to keep your deposit as
a non-refundable.*
